Here is a clue.

When I get people to clear their history it works. Seems like there is a
certain history combination that messes it up. May I suggest you try it on
a Safari browser with a lot of history and once you recreate the problem
you can sort through the history and remove it piece by piece to narrow it
down to the culprit. I don’t have time for that but you might find it worth
your while.

Update 10/25/16:

Our engineers are now alpha testing a script that would backfill missing recurring donation activities. We’ve reached out to a few customers to test these solutions, and should have more details soon.

commented on My donation page using authorize.net gives an error The supplied currency code is invalid
After troubleshooting with several customers, this error appears when the payment processor and merchant account currency codes do not align with the same currency. Customers have fixed this issue by either recreating either their Authorize account or merchant account to align with the same currency. That currency must also be reflected in NationBuilder.

For example:

1) Payment processor = CAD

2) Merchant account = CAD

3) NationBuilder currency settings = CAD
