Is there a way for adding a check box for is_private, i.e. do not publish profile, in an endorsement sign up form, so that the sign-up can have that option filled out, in turn enabling another page that shows endorsements to hide these signup entries?

Endorsement pages do not support people being able to make their endorsement private, since this page type is focused around building a list of your public supporters. However, both the Vote Pledge and Petition page types do allow people to prevent their action from being shown publicly, so these might work better if you really want people to have that option.
