We received an email from the CFB in NYC, who conducts roving audits of candidates' sites, that said this:
I reviewed donate page on your website and found that you are missing: · a way to collect the contributors employment information,
the affirmation statement,
and a required check box below the statement acting as the contributor’s signature agreeing to the affirmation statement.
I called PayPal and found that we must do this on the NB site. How can we do this?
Peter, in your control panel, go to Settings > Bank accounts and edit the account in question. You'll be able to specify requiring emploment information and add the text of the affirmation statement (which should be specific to your NYC jurisdiction) which will trigger the inclusion of the check box.
