I'm looking to create a relatively detailed form for residents to request a celebratory scroll. As far as I know you can't create an html form without php to process it, and there does not appear to be a way to upload php scripts, or some feature that would allow me to create a far more detailed contact form that would allow me to request birth dates, delivery addresses, etc...
