All forms on site not display correctly

Hi all,

I'm new to NB and hope this is a quick fix. I'm using the Aware theme.

Volunteer, Donate, and Feedback Form pages are all not showing their respective forms correctly. They're published.

Any idea what could be wrong?

Although it's been a while it looks like everything is working! We have a FAQ on form troubleshooting as well that might be helpful.
