I think that your best option would be to use the survey form page – which will allow you to customize your questions and tag the responses.

If you need a little more customization than that, you can use 123 Form Builder to build your form and embed it on a basic NationBuilder page. Hi Brian, only public pages can be indexed by Google. The only way to have membership or logged in access-only pages indexed would be to remove that restriction and make them public pages.

Hi Brian. That is not currently a functionality offered. As you’ve likely experienced, the Site Settings will set the minimum level (so “logged in” in this case) for the website. You can tighten the restrictions further for pages (i.e. members only) but not make individual pages more permissive.
