We need to have a very detailed signup form that we are using as the first page of an "application." In addition to some custom fields, we need to ask people their "demo" and "gender" are the database fields that are connected to these two issues not available as a signup variable? If not, do I just need to use custom fields?

You're correct: while "demo" and "gender" are default 'signup variables' that exist on supporter profiles within the Control Panel, they are not user-submittable. If it's important for your organization to be able to collect this information on a Signup page, then custom fields is the best way to go.
