Using Victory theme.
I need to add Employment information fields and a user agreement check box. How do I go about doing that?

Those fields are turned on in the definition of the bank account used by the donation page. You can turn them on and off by going to Settings-- Bank accounts and editing the desired account. The check boxes are in the right column of that page.
