We'd like to prepare canvassing data sheets to be filled in with pen or pencil and then be able to scan the results back into the system later.

The form only needs to record a level of support (1 to 5) and some number of yes/no answers to questions.  This is mainly a optical mark recognition (OMR) tool as would work for a multiple choice questionnaire.  The difference is that each row could be any of the thousands of people in our database. So, there needs to be some level of OCR or bar code reading to determine which person the input row belongs to.
